A routine physical involves examining your child’s immunization records and ensuring that he or she is up to date with all required immunizations. This may be information that is required for your school to admit your child in the fall. Here are a list of required school vaccinations for the state of Texas.
Diphtheria/Tetanus/Pertussis Vaccine
(DTaP/DTP/DT/Td/Tdap)
Kindergarten-6th Grade
Five doses of diphtheria-tetanus-pertussis vaccine is required for kindergarten-6th grade. One dose must have been received on or after the 4th birthday. However, only four doses is required if the 4th dose was received on or after the 4th birthday. Three doses is required for students aged 7 years and older if one dose was received on or after the 4th birthday.
7th Grade
One dose of Tdap is required if at least five years have past since the last dose of tetanus-containing vaccine.
8th Grade-12th Grade
One dose of Tdap is required when ten years have passed since the last dose of tetanus-containing vaccine. If a medical contraindication to pertussis exists, Td is acceptable in place of Tdap.
Polio Vaccine
Kindergarten-12th Grade
Four doses of polio vaccine is required. One dose must be received on or after the 4th birthday. However, three doses will meet the requirement if the third dose was received on or after the 4th birthday.
Measles, Mumps and Rubella (MMR) Vaccines
Kindergarten-12th Grade
Two doses of MMR vaccine are required, with the first dose received on or after the 1st birthday. If vaccinated prior to 2009 with two doses of measles and one dose each of rubella and mumps satisfy this requirement.
Hepatitis B Vaccine
Two doses will meet the requirement if the student is between the ages of 11-15 and received adult hepatitis B vaccine (Recombivax®) . The dosage (10 mcg/1.o mL) and type of vaccine (Recombivax®) must be clearly documented. If Recombivax® was not the vaccine your child received, a 3-dose series is required.
Varicella Vaccine
The first dose of varicella must be received on or after the first birthday. Two doses are required for kindergarten-12th grade.
Meningococcal (MCV4) Vaccine
7th-12th Grade
One dose of quadrivalent meningococcal conjugate vaccine is rquired on or after the student’s 11th birthday.
Hepatitis A Vaccine
The first dose of hepatitis A must be received on or after the first birthday. Two doses are required for kindergarten-12th grade.
